Registration form, one recent passport photograph, copy of ID / other travel document, copy of music score and bank-in slip must be received via email (info@hkomc.org), by post or in person. Late application will be subjected to an additional administration fee of HK$100.

 

請將報名表連同證件相一張，身份證 / 旅遊證件副本，樂譜影印本及銀行入數收據副本電郵至 info@hkomc.org， 郵寄或親臨以下地址遞交。逾期報名者需繳交港幣 $100額外行政費用。
